Provider Portal

What is it?

Penn State Health Provider Portal is a web-based application that provides real-time, read-only access to your patient’s information from encounters with Penn State Health.

All of the below documents must be completed and submitted in order for you practice site to begin the set-up process:

  1. AC Agreement - the Access and Confidentiality agreement defines the responsibilities and obligations of each party in regard to Penn State Health’s clinical information system.
  2. Access and Confidentiality - this is required to be signed by the user. This acknowledges an understanding that the user will not disclose, share or allow any third party to utilize the ID number and password assigned to the user. In addition, also details an agreement between the user and Penn State Health that the user will access the system to only records identified by Penn State Health.
  3. Attestation - by signing this form , the user attests that they have completed the HIPAA, Privacy and Information Protection training material document.
  4. HIPAA Training - includes key points on the need to protect the privacy and security of patients health information as mandated by the Health Insurance Portability and Accountability Act (HIPAA). This document outlines an employee's responsibility to comply with the HIPAA federal legislation and agree that any individually identifiable patient information is safe. Failure to comply with the terms may result in disciplinary action. Review this document with your supervisor or manager.
  5. Provider Portal Questionnaire - a short questionnaire to help the Health Information Management team determine the best access to meet your practice site's needs.
  6. Provider List - outlines all providers (MD/DO/CRNP/PA) who are required to have their patients available on the Provider Portal.
  7. Account Request Form - provides the Health Information Management team information needed to give an individual access to the application.
  8. Provider Portal Checklist - this checklist is to be completed by the prospective practice site Information Systems department or Practice Site Manager. This checklist outlines the Information Systems environment for a practice site and will give our staff a clear idea of the compatibility of their environment with the Provider Portal.
  9. Accellion secure dropbox - a link to securely access and share sensitive documents. Please send all documents to HIMProviderPortalGroup@pennstatehealth.psu.edu.
  1. Accellion (sending secure files) - this document will assist a user to send and request a secure file as well as send a secure message to Penn State Health. This document is intended to help a user when uploading documentation. Please send all documents to HIMProviderPortalGroup@pennstatehealth.psu.edu.
  2. Adding providers to a patient list - this document is to be used for an existing site to add a new provider to their patient list.
  3. Electronically signing a PDF - this document will assist a user to electronically sign the required documentation electronically, if they choose.
  4. Provider Portal Guide - this guide is intended to assist practice sites who are granted access use  to the Povider Portal. This guide provides instructions on the set-up and the use of the Provider Portal. The guide reviews how to view EKGs, labs, documents and scheduling to name a few features.
  5. Provider Portal PLUS Guide - this guide is intended to assist practice sites who are granted access to the Provider Portal Plus. This guide provides instructions on the set up and the use of the Provider Portal Plus. The guide reviews how to view EKGs, labs, documents and scheduling, emessaging and patient search capabilities, to name a few features.
  6. Provider Portal PLUS Video - a video that guides you through the Provider Portal PLUS.

Please note, this is only if initial site installation has already occurred.

  5. User packet - this packet allows a user to print out all the required documents for a New User to be set-up at an existing Provider Portal site in one packet.
