Urology New Patient Forms

Please print and complete the following forms and bring to your appointment.

Before your visit

We encourage questions. Before you come in, we suggest that you take a moment to write down any questions you want to ask the physician. It’s also important for us to know what medications you take and if you have any known drug allergies.

Medical records

We’ll need all of your medical records related to your urological condition at the time of your first visit. This includes any X-rays or previous surgery records. If you need to obtain X-rays or records from a doctor other than the physician who referred you, please have these sent to us before your first appointment. The process may take up to a few weeks, so request your records as soon as possible. You’ll be required to complete an Authorization for Release of Medical Records. Fax all relevant medical records to the Urology office at 717-531-4475 before your scheduled visit. For any radiology studies, have them transferred to a CD by the Radiology Department where the studies were performed. Bring the CD to your visit. If the testing has been performed at a Penn State Health Milton S. Hershey Medical Center or Medical Group location, you don’t have to bring the records with you.

Canceling your appointment

We ask for at least 24 hours notice when you need to cancel or change your appointment. This gives us the opportunity to give an appointment to a patient with an urgent need.

Pediatric Urology Patients

Our pediatric urology care is handled at the Penn State Children’s Hospital.
